Do you accept insurance for PT sessions?

At this time we are not accepting insurance, however we can create itemized statements that you can submit to your insurance company for possible reimbursement.

I have Medicare/Medicaid, can I be seen?

Due to limitations imposed by the Centers for Medicare and Medicaid services, beneficiaries are unable to participate in cash-pay or out-of-pocket therapy services provided by Animas Rehab & Performance.

What services do you offer as a physical therapist?

We provide a wide range of PT services including, but not limited to:

  • Physical Therapy
  • Post-Surgical Rehabilitation
  • Pain Management
  • Functional Dry Needling
  • Manual Therapy (e.g. joint mobilizations, soft tissue mobilization)
Are there any additional fees for PT services?

No! All services rendered during any services (evaluation, follow-up, or bike fitting) are included in the prices listed.

If the time you need is not available, you can call our clinic directly for assistance with scheduling.

What should I bring to my first appointment?

For your first appointment, please bring any relevant medical records, insurance information, and a list of medications you are currently taking to help us provide the best care possible.

What should I wear to my appointment?

Wear clothing that you are comfortable moving in, and that will allow access to the area of interest. Please wear closed toed shoes, even if the foot and ankle are the area of focus.

Examples appropriate attire include:

  • Clothing that will allow access to the shoulder (e.g. tank top).
  • Clothing that will allow access to the low back.
  • Pants that will allow access to the knee and hip.
